Purchases made through this website or in person are non-reversible and non-transferable.
5. Prescription and Insurance Responsibility
The purchase of a CPAP machine requires a valid sleep study and a prescription issued by a licensed physician. Patients are solely responsible for acquiring:
- A valid prescription;
- All required documentation necessary to submit insurance claims.
FRSC may assist in preparing supporting documents but does not guarantee any insurance reimbursement. The patient acknowledges that any failure to obtain a valid prescription or to meet insurer requirements may result in the denial of claims.
6. Appointment Scheduling and Changes
Appointment bookings made through the website are subject to availability and confirmation by FRSC staff. FRSC reserves the right to cancel, reschedule, or modify appointments due to provider availability, technical disruptions, or public health concerns. Patients are encouraged to provide at least 24 hours’ notice for appointment cancellations. Repeated no-shows may result in limited access to online booking.
7. Account Deactivation and Data Retention
You may request to deactivate your FRSC online account at any time. Deactivation will not delete medical records, which FRSC is required by law to retain for the period prescribed by Alberta’s Health Information Act. FRSC will continue to safeguard and maintain access to your records in accordance with statutory and regulatory requirements.
8. Limitation of Liability
To the maximum extent permitted by law, FRSC disclaims all liability for any damages, including direct, indirect, incidental, or consequential damages, arising out of:
- Website outages or technical issues;
- Unauthorized account access;
- Denied insurance coverage;

Patients subject to enforcement actions will be notified in writing and, where appropriate, offered an opportunity to submit a written appeal to the Clinic Director within 10 business days. Appeals will be reviewed on a case-by-case basis, and FRSC’s determination will be final.
You agree not to engage in conduct that would violate any law, infringe on third-party rights, compromise the security of the website, or misuse website content. This includes but is not limited to:
